Introduction/informed consent script for the interview:

Thank you so much for letting me ask you some questions about 5th Ward. I have three questions and this should take no more than 5 minutes of your time.

 Before we get started, I wanted to let you know that participation in this study is voluntary. 

The information we collect from these questions will help us learn more about things that affect the health and well-being of people in Fifth Ward. This will help community leaders and residents work on the things that would best help the community.

We are not asking for any personal health information from you and your answers to these questions will be anonymous.
